Getting Started with the FCHV Registry
Accessing the Platform
- Open the URL : Navigate to http://hmis.gov.np/hmis/ to access the Health Management Information System (HMIS).
- Login : Enter your HMIS username and password to sign in.
- Menu Icon : Click on the menu icon (9 dots icon) located at the top right corner of the page.
- Select FCHV APP : From the application menu, select “FCHV APP” to access the FCHV Registry.
Navigating the FCHV Registry
Registering a New FCHV
- Add Organization Unit : Click the “+” button on the top right corner of the page. This will show you a list of organization units.
- Select Organization Unit : Choose the desired health facility or organization unit from the list.
- Register Button : Click the “Register” button at the top of the page to start the registration process.
- Fill Registration Form :
- Local FCHV Number : Enter the local FCHV number in the format (Province Code/District Code/Palika Code/FCHV S.N).
- Insurance Details : Toggle the button if the FCHV has insurance and add the insurance number.
- Training Dates : Toggle the buttons to record the dates of attended trainings. You can add multiple training names by clicking the “add more training name” button.
- Upload Documents : Upload the FCHV’s photo, ID card, photo of citizenship, educational certificates, and training certificates.
- Submit : Click the submit button at the bottom of the page to complete the registration.
Managing FCHV Records
- View Details : Click the view button to see all details of a particular FCHV.
- Edit Details : Click the edit button to modify any information. After making changes, click the submit button to save.
- Deactivate FCHV : To deactivate an FCHV, edit the record and toggle the active button. Choose the reason for deactivation and submit the changes.
