History of Public Health: From Ancient Sanitation to Modern Health Systems

In short: Public health began with ancient drains, aqueducts and quarantine, grew through the sanitary movement of the 1800s, and became a science with germ theory, vaccines and modern health systems.

Ancient civilisations

The cities of the Indus Valley (around 2500 BCE) had covered drains and bathing areas. Ancient Rome built aqueducts and public baths. Greek physician Hippocrates, in 'Airs, Waters and Places', linked disease to environment and lifestyle — an early idea of disease causation beyond supernatural explanations.

Plague and quarantine

During the Black Death of the 14th century, Venice and Ragusa (Dubrovnik) introduced quarantine — the isolation of ships for 40 days (quaranta giorni). Health boards, isolation hospitals and the recording of deaths followed, laying foundations for surveillance.

The sanitary movement

Industrial cities of the 19th century were crowded and filthy. Edwin Chadwick's 1842 report on the sanitary condition of Britain's labouring population led to the Public Health Act of 1848. John Snow's work on cholera in London (1854) showed the value of mapping and data.

The scientific era

Germ theory (Pasteur and Koch), vaccines, antibiotics and modern epidemiology turned public health into a science in the late 19th and 20th centuries. The World Health Organization was founded in 1948, and the eradication of smallpox in 1980 became the field's greatest success.
